TPE net reportedly catches lots of home care providers.<\/em><\/strong><\/span><\/p>\n Scrutiny of home health and hospice claims is on the rise, and you\u2019d better know where that scrutiny may be coming from before it cuts into your bottom line.<\/p>\n In a webinar series from Medicare Administrative Contractor\u00a0National Government Services\u00a0<\/strong>titled \u201cPatients Over Paperwork \u2014 Moving Forward Together: Medicare Audit Contractors,\u201d the MAC homed in on the different audit types and how new Medicare initiatives are impacting claims review. The primary purpose of the specific auditors is to \u201creduce improper payments by identifying and addressing coverage and coding errors for all provider types,\u201d note the NGS webinar materials.<\/p>\n Remember:<\/span>\u00a0<\/strong>Though the programs have similar endpoints, \u201ceach contractor has their own way of accomplishing these goals,\u201d reminded NGS provider outreach and education consultant\u00a0Lori Langevin\u00a0<\/strong>during the webinar.<\/p>\n Take a look at this overview of the different Medicare Audit Contractors and pocket it for your records:<\/p>\n 1. Medicare Administrative Contractor.<\/strong><\/span><\/p>\n Last October, the MACs \u201ctransitioned\u201d to the Targeted Probe and Educate (TPE) strategy to review claims. Under the new program, MAC medical reviewers conduct three rounds of review, selecting 20 to 40 claims per round, and target atrisk providers as opposed to looking at 100 percent of claims. Then, Medicare providers receive a notification letter letting them know that their claims have been selected for an audit. The notice \u201cwill provide you all the details about your particular TPE,\u201d Langevin says.<\/p>\n This type of audit may impact home health agencies most widely, anecdotal evidence suggests. Multiple home health billing services says\u00a0<\/strong>that a large ratio of their clients are being put under the program.<\/p>\n MACs now do TPE claims reviews specifically \u201cto reduce costs related to improper payments and appeals,\u201d NGS says. The new process is meant to reduce provider burden, suggests Langevin. \u201cIt [TPE] is a better process not just for contractors but for providers, too \u2014 more involvement, more education, and more 1-on-1.\u201d She adds, \u201cThe providers seem to like that feedback.\u201d<\/p>\n The MAC decides how many claims your organization must furnish and when to send them. As with start dates, providers\u2019 end dates for TPE rounds will vary due to this. The\u00a0Centers for Medicare & Medicaid Services\u00a0<\/strong>allows 45 days to respond to the ADR, but NGS recommends you respond in 30 \u2014 \u201cclaims will deny on day 46 if the records are not received,\u201d the NGS webinar materials state.<\/p>\n Postpayment:<\/span>\u00a0<\/strong>Though TPE is a prepayment\u00a0review process, the MACs may still request provider\u00a0records for post-pay probes, the NGS materials suggest.<\/p>\n \u201cWe aren\u2019t saying that we are never going to do postpayment [reviews], but our goal is to keep it prepay leaning,\u201d Langevin counsels.<\/p>\n Tip:<\/span>\u00a0<\/strong>\u201cDo not send documentation until you get your additional development request (ADR),\u201d Langevin cautions. \u201cIt will include a list of specific elements needed to support the services we are reviewing.\u201d<\/p>\n 2. Comprehensive Error Rate Testing (CERT) Auditors<\/strong><\/span><\/p>\n The CERT audits are exclusively post-payment reviews. \u201cThe CERT program was designed to determine if Medicare contractors are processing and paying claims correctly,\u201d says\u00a0Laura Brown,\u00a0<\/strong>an NGS provider outreach and education consultant. Why does this apply to you? If the CERT finds errors involving money overpaid to your agency, it instructs your MAC to recoup the funds from you. In addition, errors that the CERT identifies can become issues of focus in future MAC and RAC audits.<\/p>\n Warning:\u00a0<\/strong>One more thing \u2014 your error rate is picked up by the\u00a0Medicare Payment Advisory Commission<\/strong>, the\u00a0HHS Office of Inspector General<\/strong>, the\u00a0Government Accountability Office<\/strong>, and other federal organizations to be used to target you for further scrutiny outside CMS.<\/p>\n You must put the highest priority on sending documentation back to the CERT auditors, Brown warns. \u201cIf you do not return documentation immediately, you will receive telephone contacts and letters about every 10 to 15 days for approximately 75 days.\u201d She adds, \u201cIf you don\u2019t respond by the 75th<\/sup>\u00a0day, the claim will be canceled and any Medicare pay will be recouped.\u201d<\/p>\n Tip:\u00a0<\/strong>If an auditor asks for records from you, double-check that you\u2019re submitting every document she requested. Lack of documentation is a top finding in CERT reviews.<\/p>\n 3. Recovery Audit Contractors (RACs)<\/strong><\/span><\/p>\n The RACs do only postpayment reviews and follow the same review policies as the MACs. \u201cThey have to abide by National Coverage Determinations (NCDs), Local Coverage Determinations (LCDs), and CMS manuals and regulations just like NGS does,\u201d Langevin stressed. \u201cWe just want providers to know that they [RACs] aren\u2019t looking at your claim any differently than we would.\u201d<\/p>\n Interesting:\u00a0<\/strong><\/span>\u201cThe RACs don\u2019t just have anyone review the claims. They need to be well-qualified,\u201d says Langevin. The RACs use \u201cnurses, therapists, certified coders, and a contractor medical director\u201d for thorough and accurate reviews, notes the NGS guidance.<\/p>\n The auditors can look back for three years from the date your claim was paid with the option to perform two types of reviews. These include either an automated review, which looks specifically at previously submitted Medicare claims, or a complex review, where the auditor will request records from your agency.<\/p>\n Time frame:<\/span>\u00a0<\/strong>Once the process starts, RACs have 30 calendar days to review your information and return a decision to you. The MAC in your jurisdiction will receive the adjustment from the RAC 30 days after the initial determination letter was sent.<\/p>\n 4. Zone Program Integrity Contractor (ZPIC) or Unified Program Integrity Contractor (UPIC)<\/strong><\/span><\/p>\n The ZPICs and UPICs investigate potential Medicare fraud based on specific circumstances using data analysis, interviews, documentation, records, and onsite visits, indicate the NGS materials.<\/p>\n After their reviews, which do include looking into fraud and abuse, they can withhold Medicare payments and refer fraudsters to law enforcement, NGS says.<\/p>\n Important:<\/span>\u00a0<\/strong>ZPICs\/UPICs \u201csupport victims of Medicare identity theft,\u201d relays Langevin. \u201cIf a provider believes he or she may have had their information stolen or used to submit Medicare claims, they can request a ZPIC\/UPIC from their MAC.\u201d<\/p>\n Source- SuperCoder<\/span><\/i><\/a><\/p>\n","protected":false},"excerpt":{"rendered":"
