Federal prosecutors filed criminal charges in 439 cases involving 720 defendants, with a total of 639 defendants were convicted of health care fraud-related crimes during the year. The DOJ also opened 948 new civil health care fraud investigations and had 1,086 civil health care fraud matters pending at the end of the year, the report says.<\/p>\n<p>On the OIG side, investigations resulted in 788 criminal actions against individuals or entities and 818 civil actions, which include false claims and administrative recoveries related to provider self-disclosure matters. The OIG also excluded 3,244 individuals and entities from participation in Medicare, Medicaid, and other federal health care programs.<\/p>\n<p>The report reiterates high-profile fraud cases for\u00a0<strong>Passages Hospice\u00a0<\/strong>in Illinois related to GIP overbilling and kickbacks;\u00a0<strong>Hospice Plus<\/strong>,\u00a0<strong>Goodwin Hospice<\/strong>, and\u00a0<strong>Phoenix Hospice\u00a0<\/strong>in Texas paying kickbacks for referrals; physician\u00a0<strong>Nathaniel Brow<\/strong>n taking kickbacks for bogus referrals to\u00a0<strong>Milestone Hospice\u00a0<\/strong>and\u00a0<strong>Sandanna Hospice\u00a0<\/strong>in Mississippi;\u00a0<strong>Home Care Hospice Inc.\u00a0<\/strong>in Pittsburgh and its owner\u00a0<strong>Matthew Kolodesh\u00a0<\/strong>and codefendants billing for services that were unnecessary or not provided and overbilling for GIP; long-term care company\u00a0<strong>Genesis Healthcare Inc.\u2019s\u00a0<\/strong>$53.6 million payment to settle hospice and therapy overbilling charges; and Ohio-based\u00a0<strong>Foundations Health Solutions Inc.<\/strong>,\u00a0<strong>Olympia Therapy Inc.<\/strong>, and\u00a0<strong>Tridia Hospice Care Inc.<\/strong>, and two of their executives\u2019 $19.5 million payment to settle allegations of false claims for medically unnecessary rehab therapy and hospice services.<\/p>\n<p>The report also notes that the OIG\u2019s Civil Division\u2019s Commercial Litigation Branch (Fraud Section) \u201chas continued to pursue claims against hospice providers for billing Medicare for patients who did not have a terminal prognosis of six months or less or who received medically unnecessary services.\u201d<\/p>\n<p><em>Note: See the 95-page report at\u00a0<a href=\"https:\/\/oig.hhs.gov\/publications\/docs\/hcfac\/FY2017-hcfac.pdf\">https:\/\/oig.hhs.gov\/publications\/docs\/hcfac\/FY2017-hcfac.pdf<\/a>.<\/em><\/p>\n<p><a href=\"https:\/\/www.supercoder.com\/coding-newsletters\/my-hospice-insider-alert\/fraud-abuse-fraud-report-tallies-hospice-enforcement-activities-157398-article\"><i><span style=\"font-weight: 400;\">Source- SuperCoder<\/span><\/i><\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>High-profile cases paint industry with fraud brush.
