Under PAC PPS, home health stays would see downward adjustment.<\/em><\/strong><\/span><\/p>\n While home care providers may have more pressing matters to deal with, they shouldn\u2019t lose sight of the\u00a0Medicare Payment Advisory Commission\u2019s\u00a0<\/strong>efforts at Congress\u2019 behest to work on a payment system spanning four post-acute settings.<\/p>\n A unified Post-Acute Care Prospective Payment System \u201ccould use readily available data to pay for a stay based on a patient\u2019s characteristics, not the site of service or the amount of therapy furnished,\u201d MedPAC maintains in its March report to Congress. \u201cThe design would correct current distortions in the SNF and HHA PPSs that encourage providers to furnish services of questionable value and advantage providers that avoid medically complex patients.\u201d<\/p>\n Before beginning the switch to the new PAC PPS in 2021, Congress should direct the\u00a0Centers for Medicare & Medicaid Services\u00a0<\/strong>to \u201cbegin blending the relative weights of the setting-specific payment systems and the unified PAC PPS in 2019,\u201d the commission urges. \u201cUnder this blend, each PAC setting\u2019s total payments would be kept at the recommended level while payments would be redistributed within each setting based on a provider\u2019s mix of patients, costs, and therapy practices.\u201d<\/p>\n In its March meeting, MedPAC staffer\u00a0Carol Carter\u00a0<\/strong>noted that \u201csequential stays\u201d for home health patients differed from those in the other three settings (skilled nursing facility, inpatient rehab facility, and long-term care hospital). Costs were lower for HHAs than for the other provider types.<\/p>\n \u201cWithout an adjustment, profitability under a PAC PPS would be higher for later home health stays,\u201d Carter noted in the presentation.<\/p>\n The unified PPS already would have a large adjustment for home health agencies, note\u00a0Daniel Kim\u00a0<\/strong>and\u00a0Olivia Seraphim\u00a0<\/strong>with law firm\u00a0Epstein Becker Green\u00a0<\/strong>in analysis published on its website.
